You seem to disregard or not realize that there are other motivations than legislation to get people to buckle up. For example, if you are not wearing your seatbelt, get in a crash, and bounce around the car injuring passengers, then you should be liable for their medical costs. If you kill them, you should be charged with manslaughter.
Another example, if you are driving alone, not wearing seatbelt, cause a crash, and get injured that a seatbelt would prevent, the insurance company could have a condition in the policy that states you are liable for half (or some percentage) of the medical costs.
